Catholics don't believe they earn salvation.

It is the Vatican's official position that a person partake in the event, and they can lose it. Therefore, they earn salvation.

If no human cooperation is necessary and no action according to human will is meaningful, why isn't everyone saved?

The only alternatives are to either maintain that God simply can't be bothered to communicate his grace to those who aren't saved, or that he actively wills their damnation.

Who said anything about cooperation? We are talking good works. The Lord Jesus Christ said to enter into life, keep the commandments. Do you deny he said that?

Fist you have to have total faith in the risen Christ, I mean bone deep total! Then you receive Grace, and Grace, or the Holy Spirit, if you prefer, guides you into a better will and happiness, so what you do is good because you are in Christ. Before Grace, you have no hope unless you believe in Him who saves. The rest is easy.
