How do you imagine? The Bible says a married man, not a eunuch, as that is taking scripture out of context.
But that never happened. As for hierarchy every man reports directly to Jesus Christ. There are no biblical middle men.
The Catholic church became universal through syncretism, some of this and some of that to appeal to everyone.

The Protestants believe the scriptures and do not worship idols, the Catholics ignore the scriptures and make many idols to worship.
